BIRTH ANNOUNCEMENT

* Birth announcements for parents with two different last names must have a waiver of information form completed prior to publication of birth announcement.

This form must be notorized by an authorized Notary of Public, State of Iowa.

Please stop by the Maquoketa Sentinel-Press office located at 108 W. Quarry St., Maquoketa, IA to pick up your form and have it notorized on the spot at no cost. Please bring a form of ID with you.

Please download and fill out the form below and send as an attachment to newsdept@mspress.net. Before attaching to an email, make sure you have saved the document and are attaching the correct one! Also, be sure to include your photograph if you wish to have it published, too.
